I received the information about including my personal data to the data base as well the rights provided by the personal data protection laws and the purpose of data collection as well as the persons who receive the personal data. I am aware of the rights I have according to the Art.38 oh the Law of Ukraine "About personal data protection". I do not have any objections, comments, or complaints.
